For too long, periodontal disease and dental caries have been reduced to simple infection stories, when the clinical reality is far more dynamic. Oral disease develops through a complex interaction between microbial shifts, saliva, biofilm structure, inflammatory signaling, and the host response that determines whether tissues remain resilient, become inflamed, or begin to break down. This course uses phase-contrast microscope slides, salivary diagnostics, medical history review, and chairside testing to help clinicians recognize patterns of health, dysbiosis, active inflammation, caries risk, and advanced disease susceptibility. Participants will explore how organisms such as spirochetes, motile rods, amoebae, dense biofilm communities, and inflammatory cells reflect a changing oral ecosystem—but also why the patient’s immune response, systemic health, medications, hormones, airway, nutrition, and stress load influence the outcome. Saliva, pH, buffering capacity, dry mouth, bleeding, microbial testing, and inflammatory markers will be discussed as practical tools for identifying risk before destruction progresses. Attendees will leave with a deeper understanding of oral disease as a dynamic conversation between microbes, saliva, terrain, immune resilience, and whole-body health—and with practical ways to guide patients toward prevention and healing.

After this course attendees will be able to:

  • Explain how oral disease reflects a dynamic interaction between the microbiome, biofilm behavior, saliva, immune response, and systemic health
  • Contrast the traditional infection-based model of periodontal disease and dental caries with a host-response and ecosystem-based model of disease progression
  • Describe how phase-contrast microscopy can help clinicians visualize patterns associated with health, dysbiosis, inflammation, caries risk, and periodontal disease
  •  Identify key microscopic findings, including cocci, rods, spirochetes, motile organisms, amoebae, white blood cells, and biofilm density, and relate these findings to clinical risk
  • Discuss how saliva, pH, buffering capacity, dry mouth, microbial testing, and inflammatory markers can be used to better understand oral disease susceptibility
  • Evaluate the role of the medical history, including medications, hormones, airway, nutrition, stress, immune status, and systemic conditions, in shaping the oral environment and host response
  • Integrate microscopy, salivary diagnostics, chairside testing, and host-response thinking into patient education, prevention strategies, and personalized oral-systemic care
